Subject: SUBJECT: Official State Prescription Form (Re-introduction)
 
In the near future, I plan to reintroduce legislation – former House Bill 2412 of the 2015-16 Legislative Session – authorizing the establishment of an official state prescription form.

Under my proposal, controlled substances in Schedules II, III, IV, and V must be prescribed or dispensed only on a serialized state prescription form issued by the Department of Health, unless prescribed electronically. Additionally, practitioners would be required to maintain a record of the disposition of the forms as well as notify the department in cases of loss, theft, or unauthorized use.

Enactment of an official state prescription form will help deter fraudulent prescriptions, prevent opioid abuse, and protect public health in our Commonwealth. Therefore, I encourage your consideration and co-sponsorship.
