4(E) No outpatient provider, other than a licensed pharmacy,
5may seek reimbursement for drugs dispensed in excess of a
6fifteen-day supply during the first thirty days of treatment for

9drugs commencing upon the employe's initial treatment with such
10outpatient provider. If the employe's injury recurs or is
11reevaluated after the initial course of treatment, an outpatient
