Amend Bill, page 3, line 23, by inserting after "dependents"

 and indicate whether the claimant qualified for senior citizens

property tax and rent rebate assistance as provided in Chapter

13 of the act of June 27, 2006 (1st Sp.Sess., P.L.1873, No.1),

known as the "Taxpayer Relief Act," or for pharmaceutical

assistance for the elderly, as provided in Chapter 5 of the act

of August 26, 1971 (P.L.351, No.91), known as the "State Lottery

Law," during the prior taxable year
