AN ACT

Prohibiting the use of full Social Security numbers on State

2

forms.

The General Assembly of the Commonwealth of Pennsylvania

4

hereby enacts as follows:

Section 1.  Short title.

This act shall be known and may be cited as the Social

7

Security Number Prohibition Act.

Section 2.  Use of full Social Security numbers prohibited.

Notwithstanding any other law to the contrary, except as

10

required by Federal law or regulation and except as provided in

11

section 3, no person shall be required to provide more than the

12

last four digits of his Social Security number on any form

13

required by a State governmental entity.

Section 3.  Exceptions.

15

This act shall not apply to:

16

(1)  Forms required by the act of March 4, 1971 (P.L.6,

No.2), known as the Tax Reform Code of 1971.

(2)  Any other form utilized in connection with the

3

collection of State taxes.

Section 4.  Effective date.

5

This act shall take effect in 90 days.
