AMENDMENTS TO HOUSE BILL NO. 1170

Amend Bill, page 3, line 20, by inserting after "longer."
An employer shall notify the department of the employer's
initial compliance with this subsection. The notice shall be on
a form prescribed by the department and shall be made available
on the department's publicly accessible Internet website. The
employer shall include all of the following information in the
notice:
(1) The employer's name.
(2) The employer's mailing address, including each
physical location of the employer within this Commonwealth.
(3) The names and mailing addresses of each owner,
partner or officer of the employer.
