Amend Bill, page 1, line 3, by inserting after "record"
and providing for non-certified copy of original birth record
Amend Bill, page 1, lines 6 through 9, by striking out all of
said lines and inserting
Section 1. Section 2911 of Title 23 of the Pennsylvania
Consolidated Statutes is amended by adding a definition to read:

of said lines
Amend Bill, page 2, line 13, by striking out the bracket
before "(a)"
Amend Bill, page 2, line 13, by inserting a bracket before

] For adoptees specified under subsection (d), no

Upon request by either birth parent, the Department of
Health shall also provide the birth parent with an updated
medical history form, which shall be completed and returned
by the birth parent to the Department of Health.

(d) Applicability.--This section shall apply to persons
placed for adoption before the effective date of this

Section 3. Title 23 is amended by adding a section to read:
Section 2937.1. Non-certified copy of original birth record.
Amend Bill, page 8, by inserting between lines 16 and 17
(h) Applicability.--This section shall apply to persons
placed for adoption on or after the effective date of this
section.
Amend Bill, page 8, line 17, by striking out "3" and
inserting

Amend Bill, page 8, lines 18 and 19, by striking out all of
said lines and inserting
(1) The following provisions shall take effect in one
year:
(i) The amendment of 23 Pa.C.S. § 2937.
(ii) Except as set forth in paragraph (2), the
addition of 23 Pa.C.S. § 2937.1 shall take effect in one
year.
