A RESOLUTION
Urging the Department of Transportation to change its policy
UPDATE ITS REGULATION on the documentation required for a
name change to a driver's license or identification card.
WHEREAS, The Department of Transportation has an established
policy as to the type of identification required for a change of
name to a driver's license or identification card; and
WHEREAS, An individual who adopts the surname of her the
individual's spouse is only required to provide a copy of the
couple's marriage certificate to obtain a change of name on her
a noncommercial driver's license or an identification card; and
WHEREAS, In contrast, an individual who adopts the surname of
her the individual's spouse and retains her the individual's
former surname as a middle name must provide a copy of the
individual's Social Security card, together with copies of
documents from two other sources issued in the desired name,
such as tax records, Selective Service card, voter registration
card, passport, photo identification issued by a governmental

agency or state-issued birth certificate; and
WHEREAS, Individuals who retain their former surnames find it
difficult to obtain the additional required documentation; and
WHEREAS, Requiring this additional documentation constitutes
an unfair and unreasonable practice by the Department of
Transportation; therefore be it
RESOLVED, That the House of Representatives urge the
Department of Transportation to change its policy of requiring
UPDATE ITS REGULATION WHICH REQUIRES difficult-to-obtain
documentation to effect a change of name to the driver's license
or identification card of an individual who adopts the surname
of her the individual's spouse and retains her the individual's
former surname as a middle name; and be it further
RESOLVED, That the House of Representatives urge the
Department of Transportation, for a name change to the driver's
license or identification card of an individual who adopts the
surname of her the individual's spouse and retains her the
individual's former surname as a middle name, to accept the
individual's marriage certificate along with her and Social
Security card as sufficient documentation; and be it further
RESOLVED, That a copy of this resolution be transmitted to
the Secretary of Transportation.
