AN ACT
1 Amending the act of June 30, 1995 (P.L.170, No.25), entitled "An
2 act providing for voter registration, for registration
3 commissions, for remedies and for absentee ballots; imposing
4 penalties; making appropriations; and making repeals,"
5 further providing for the application form.
6 The General Assembly of the Commonwealth of Pennsylvania
7 hereby enacts as follows:
8 Section 1. Section 527(a)(1) of the act of June 30, 1995
9 (P.L.170, No.25), known as the Pennsylvania Voter Registration
10 Act, is amended to read:
11 Section 527. Preparation and distribution of applications.
12 (a) Form.--
13 (1) The secretary shall prescribe the form of official
14 voter registration application. The official voter
15 registration application shall provide space for the
16 following information about the applicant:
17 (i) Full name.
18 (ii) Address of residence. If the residence is a
1 portion only of the house, the location or number of the
2 room, apartment or floor which is occupied.
3 (iii) Mailing address if different than address of
4 residence.
5 (iv) Name and residence address on previous
6 registration and the year of that registration.
7 (v) Designation of political party, for the purpose
8 of voting at a primary election.
9 (vi) Date of birth.
10 (vii) Telephone number. An application shall not be
11 rejected because of noncompliance with this subparagraph.
12 (viii) Race, including, but not limited to, black,
13 white or Hispanic. An application shall not be rejected
14 because of noncompliance with this subparagraph.
